PRENATAL CONSENT FORM

The midwives practicing at Family Nurse Midwife Associates will provide you with prenatal care, labor management and will do vaginal deliveries for normal healthy pregnancies. All care in the office will be provided by a Certified Nurse Midwife who is available to you on a 24 hour call basis by either contacting the office or by having her paged through the hospital operator. Dr. Kahlil or one of his associates is available to the midwife for consultation or referral at all times. All prenatal patients are seen by Dr. Kahlil at about 30 weeks gestation or earlier if indicated. If a complication should develop during your pregnancy, the midwife will consult with the physician. In the case of a serious medical complication, you may be asked to transfer to a physician for the remainder of your pregnancy and delivery. The midwife and her staff will assist you with this.

 

The initial prenatal visit will include a complete history and physical, a pelvic exam and time for education on diet and exercise. Your initial lab work will be ordered and a sonogram may be scheduled if indicated. Drug testing may be ordered at the discretion of the midwife or physician without additional consent beyond this form. The midwife will see you prenatally, assist with your labor and deliver the baby. The physician will be consulted in labor if a complication develops and the delivery will take place at Jeannette District Memorial Hospital. The midwife will perform the initial exam on the infant but a pediatrician or family practice will need to be chosen by you to take care of the infant while in the hospital and to discharge the baby. The midwife will discharge you from the hospital.

 

It is the Patient's responsibility to see the midwife regularly, as advised, and to notify the office in advance if you need to cancel and to reschedule. Failure to be seen for care on a regular basis may require transfer to a physician as your risk is increased. You should follow the plan of care provided by the midwife a closely as possible. All appointments should be kept with Dr. Kahlil. Childbirth classes are encouraged along with home study; use of our library is encouraged. You should identify a physician on staff for management of the newborn and see the midwife for post delivery care.
